Role Overview
Avail Health is building a behavioral health and dementia care program supporting Medicare patients with serious mental illness, complex psychiatric conditions, substance use disorders, and dementia-related needs. The Psychiatric Mental Health Nurse Practitioner (PMHNP) is the clinical psychiatric anchor of that program — delivering comprehensive psychiatric evaluation, medication management, and dementia care via telehealth within a highly coordinated interdisciplinary team.
In addition to direct clinical care, the PMHNP serves as the GUIDE Provider of Record for Avail Health's participation in the CMS GUIDE (Guiding an Improved Dementia Experience) model, providing clinical oversight and serving as the authoritative clinical voice within the program's dementia care framework.
You'll work closely with the LCSW, Dementia Care Navigator, and Care Coordinator, supported by an ambient AI scribe and technology-enabled workflows designed to reduce administrative burden and support high-quality virtual care delivery.
This is a foundational clinical role in a newly launching program.
What You'll Own
• Psychiatric evaluation, medication management, and clinical decision-making for a complex adult and geriatric Medicare patient panel
• Clinical oversight of the CMS GUIDE dementia care program as Provider of Record
• Evidence-based SUD diagnosis and medication-assisted treatment in coordination with the LCSW
What You'll Do
• Conduct comprehensive psychiatric diagnostic evaluations including history-taking, mental status examination, and DSM-5 diagnostic formulation across mood, anxiety, psychotic, trauma-related, and late-life psychiatric presentations
• Prescribe, titrate, and monitor psychotropic medications with attention to geriatric pharmacology, polypharmacy risks, drug-drug interactions, and comorbid medical conditions
• Assess, diagnose, and provide pharmacological treatment for substance use disorders including initiation, titration, and monitoring of medication-assisted treatment with buprenorphine/naloxone
• Evaluate and manage patients across the dementia spectrum, including pharmacological management of behavioral and psychological symptoms of dementia (BPSD)
• Serve as GUIDE Provider of Record — providing clinical oversight for care plans, partnering with the Dementia Care Navigator and LCSW, and contributing to dementia care protocol development in alignment with CMS requirements
• Support patients transitioning from inpatient psychiatric or medical settings, including medication reconciliation, psychiatric stabilization, and care continuity
• Conduct clinical risk assessments, develop individualized safety plans, and coordinate escalation with care team members and emergency services when indicated
• Participate in interdisciplinary huddles and case review with the LCSW, Dementia Care Navigator, Care Coordinator, and clinical leadership
• Complete all clinical documentation accurately within 48 hours of encounter, meeting applicable regulatory, clinical, and billing standards
• Contribute frontline clinical insight to support workflow refinement, protocol development, and scalable program infrastructure as the market grows

• Post-hospitalization transitions supported seamlessly, with psychiatric stabilization and community reintegration on track
• Documentation completed accurately and on time, meeting all compliance and billing requirements
What You Bring
Required:
• MSN or DNP with a specialization in psychiatric mental health nursing from an accredited program
• Active, unrestricted APRN licensure in the State of Maryland in good standing
• Board certification as a PMHNP-BC through ANCC or equivalent certifying body
• Active Maryland DEA registration including Schedule III–V controlled substance prescribing authority
• Minimum 5 years of post-certification clinical experience in psychiatric advanced practice with demonstrated expertise in psychiatric evaluation, medication management, and geriatric mental health
• Extensive clinical experience in dementia evaluation and management, including pharmacological management of BPSD
• Clinical experience in SUD diagnosis and treatment, including initiation and monitoring of medication-assisted treatment
• Experience ordering and coordinating home health services including behavioral health nursing, PT, and skilled nursing
• Proficiency with EHRs and telehealth platforms; comfort with technology-enabled clinical workflows
• Reliable high-speed internet and a dedicated, HIPAA-compliant home workspace
Preferred:
• Fluency or conversational proficiency in Spanish
• Fellowship or advanced training in geriatric psychiatry
• Experience as a provider of record within a CMS Innovation Center model or similar value-based care program
• Familiarity with the CMS GUIDE model or other dementia-focused care navigation programs
• Familiarity with Medicare Advantage, TCM, or value-based care models
• Experience with ambient AI scribe tools or other technology-enabled clinical platforms
• Experience within startup healthcare organizations, new program launches, or rapidly scaling care environments
Schedule and Work Style
Work Type: Fully remote — virtual telehealth delivery
Schedule: Monday–Friday, 8:00 AM – 5:00 PM ET
Travel: Preference given to candidates residing in Maryland or within commuting distance for occasional in-person meetings or program activities

Compensation and Perks
This role is structured as a part-time hourly position during the initial program launch phase, with a target transition to full-time salaried status as patient panel volume grows. The anticipated full-time annualized salary range is $150,000–$170,000 ($72–$82/hour), commensurate with experience. Benefits are available from day one.
