Role Overview

Avail Health is launching a hospital-based Care Transitions Program supporting Medicare patients with complex medical, behavioral health, and social needs following discharge. The Senior Manager, Clinical Operations is the operational backbone of that program — the primary ops partner to the Senior RN Manager and the leader responsible for building, implementing, and stabilizing the workflows, reporting, and coordination infrastructure that makes the program run.
This is a fully remote role. You’ll oversee the Care Coordinator team, manage operational workflows across scheduling, referral coordination, telehealth readiness, and patient outreach, and partner with clinical and executive leadership on everything from staffing models to dashboard development. The program is being built in real time alongside hospital discharge workflows — processes will evolve rapidly and you’ll be the one adapting them.
This is a ground-floor ops leadership opportunity. You’re not optimizing an existing program — you’re building one.

What You'll Own

•   Operational infrastructure, workflow development, and day-to-day execution of the Care Transitions Program
•   Leadership, onboarding, coaching, and performance oversight of the Care Coordinator team
•   Reporting, dashboards, escalation pathways, and operational KPI monitoring across the program

What You'll Do

•   Lead development, implementation, and scaling of interdisciplinary workflows, staffing models, escalation pathways, and care coordination operations
•   Partner closely with the Senior RN Manager on day-to-day operations: workflow oversight, scheduling, staffing coordination, onboarding, and escalation management
•   Provide direct leadership and operational oversight for the Care Coordinator team: training, workflow audits, productivity monitoring, and performance coaching
•   Monitor CMS TCM workflow timelines, patient throughput, outreach completion, referral coordination, and operational KPIs; identify and address barriers proactively
•   Build and refine reporting structures, dashboards, and communication pathways that give clinical and executive leadership visibility into program performance
•   Collaborate with technology, analytics, and executive teams to develop scalable operational infrastructure supporting Care Transitions and future care management programs
•   Identify workflow gaps, staffing risks, and process failures; adapt quickly as hospital partnership needs and patient complexity trends evolve
•   Partner with executive leadership on program performance, operational strategy, and scaling initiatives

•   Care Coordinators demonstrate strong accountability, responsiveness, and consistent workflow execution with minimal operational breakdowns
•   Scheduling, outreach, referral coordination, and telehealth workflows operate efficiently and consistently across the patient panel
•   Clinical leadership feels operationally supported and can stay focused on patient care rather than coordination logistics
•   Operational infrastructure and reporting remain scalable and aligned as the program grows
•   Team members feel organized and supported within a fast-moving startup environment

What You Bring

Required:
•   5+ years of healthcare operations, care management operations, population health operations, or related healthcare leadership experience
•   2+ years of experience leading or managing operational staff or interdisciplinary support teams in a healthcare environment
•   Demonstrated experience building scalable workflows, operational infrastructure, and reporting systems in fast-paced or evolving healthcare settings
•   Experience supporting complex healthcare workflows: interdisciplinary coordination, scheduling operations, referral management, patient engagement, or transitions of care
•   Strong technology proficiency: EMRs, dashboards, scheduling tools, reporting systems, and communication platforms
•   High degree of operational autonomy, adaptability, and comfort building in ambiguous, rapidly evolving environments
•   Reliable high-speed internet and a dedicated, HIPAA-compliant home workspace

Preferred:
•   Experience in TCM, hospital discharge workflows, post-acute care coordination, or readmission reduction programs
•   Experience supporting Medicare-aged or medically complex populations including behavioral health or dual-diagnosis populations
•   Experience with virtual care, telehealth, mobile care delivery, or managing remote interdisciplinary workforces
•   Background in startup launches, program implementation, or operational redesign in healthcare

Schedule and Work Style

Work Type: Fully remote / work from home
Schedule: Monday–Friday, 8:00 AM – 5:00 PM ET with flexibility for operational needs
Travel: Occasional travel for leadership meetings, hospital partnership activities, offsites, or onboarding
Autonomy: High — senior leadership role with direct executive partnership and significant ownership of program operations

Compensation and Perks

Salary Range: $105,000 – $140,000 annually, commensurate with experience
